What to avoid with essential tremors?

You cannot prevent essential tremor but you may be able to minimize symptoms by limiting or avoiding triggers. Essential tremors can be worse when a person is experiencing stress or not sleeping enough, smoking, drinking caffeine and/or alcohol, and taking certain medications.

Its use in the treatment of tremor of the upper extremities is limited because it commonly causes weakness. It is more useful in the treatment of head tremor because it often provides benefit without unwanted, troublesome weakness. Many other medications have been reported to be of benefit in the alcohol and essential tremor treatment of essential tremor. Most of the evidence, however, has come from case reports and small, open-label studies. The chosen medication, primidone or propranolol, is introduced at a low dose and increased slowly until sufficient benefit is achieved or the usual maximum dosage is reached.

A surgeon then inserts an electrode into the area and an implantable pulse generator under the skin of the chest. The generator helps control the tremor by sending electric currents to the brain. There may be genetic factors involving specific chromosomes or the nervous system, but research has not yet confirmed this. According to the National Organization for Rare Disorders (NORD), 17–50% of essential tremor cases may be hereditary.

If it affects the quality of life or ability to do everyday tasks, a doctor may recommend one of the following approaches. In a randomized, double-blind, crossover study, tremor was reduced significantly by clozapine in 13 of 15 patients with drug-resistant essential tremor. The investigators compared a single 12.5-mg dose of clozapine with placebo. Benzodiazepines, particularly clonazepam and alprazolam, are used commonly in the treatment of essential tremor, but their effectiveness is limited. Common adverse effects include sedation, dizziness, and ataxia at higher doses. Average tremor reduction is 50–70%, but while some patients experience marked tremor reduction, others derive no benefit from the drug.

Classification: Postural, Rest and Action Tremors

Stereotactic thalamotomy is less expensive than deep brain stimulation, no hardware remains, and it has been demonstrated to provide long-term efficacy. Potential adverse effects include intracerebral hemorrhage, motor weakness, dystonia, speech disturbance, and memory loss. Alcohol withdrawal tremor is similar to essential tremor on examination but with subtle differences. In one study,10 74 percent of the patients with alcohol withdrawal tremors had tremors at a frequency above 8 Hz. In this same series, tremors in all of the patients who had essential tremor were at a frequency below 8 Hz. Thus, the tremor of alcohol withdrawal tends to be more rapid than essential tremor.

Delirium tremens is a much more serious form of alcohol tremors and usually appears a couple of days after someone ends an intense drinking binge. DT’s are especially common if you do not eat enough during your drinking binge or have a long history of alcoholism. Pahwa et al reported good results with bilateral thalamic deep brain stimulation in 9 patients with essential tremor. Patients experienced 68% improvement in hand tremor following the first surgery and 75% improvement in the opposite hand following the second surgery.
